Download the Exam Form to complete your 2024 health screening.
- Complete the top half of the form.
- Have your primary care provider complete the tobacco usage and preventive exam sections, and sign the form.
- Your provider may use the waiver section if applicable.
- Return to the screening management site to upload your completed form.
All exam forms must be uploaded by September 30, 2024.
Spouses must create their own accounts. Please only submit your exam form under your own personal account. If the participant name on the form does not match the account owner, your form cannot be accepted.
